Your information
Healthhubsupport collects your contact details for recruitment. If shortlisted, we request identity, right-to-work, training and screening evidence. Requested evidence may include criminal-history or health information needed for your role.
Authorised staff check your information with relevant screening authorities, such as the WA Department of Communities, ACIC and the NDIS Commission, where needed. Approved onboarding information may be entered into ShiftCare. This website stores and processes records using AWS in Sydney.
Send only what the recruitment team requests. Read the recruitment privacy notice before agreeing.
Why we ask
We use the information you provide to contact you about recruitment, assess suitability for the defined role, record human checks and prepare approved onboarding information. Uploading a document does not verify a clearance or approve an application.
Your choices
Consent is not pre-selected. If you cannot provide a requested document, or need help applying, contact the team to discuss an assisted process. Missing required evidence may prevent the application progressing.
Access, correction and privacy questions
Call +61 8 9228 1600 to request access or correction, raise a privacy concern or discuss withdrawing your application. Do not send identity or health documents by email.
Retention
The provider is confirming the exact retention and deletion schedule, including treatment of successful, rejected and withdrawn applications and any legal holds. Real collection will not open under this draft notice.
